Medical Oxygen in Africa: Build a Maintenance Service Hospitals Can Sustain

Medical oxygen infrastructure creates a potential service opportunity for equipment manufacturers, engineering firms and commercial partners in Africa. The useful starting question is not simply how many plants a country needs, but who will keep a specific installation working, supply its approved spare parts and fund its operating costs over time.

Verified developments: services alongside equipment

Unitaid’s 26 February 2026 announcement of the four-year INNOVATE initiative described pilots in India, Kenya, Malawi, Nigeria and Senegal. Beyond testing technology, the project plans to examine spare-parts supply, cylinder and mini-tank distribution, and outsourced plant operation and maintenance. These are activities to be tested, not evidence that a particular business model has already succeeded or that a supplier contract is open.

Separately, Unitaid’s project page, checked on 6 November 2026, lists its East African oxygen-production programme as active, with a 2024–2027 timeframe and a US$22 million grant. It describes a production-and-distribution network linking larger hubs with smaller facilities. A programme’s status does not establish the commissioning date or purchasing needs of every individual site.

WHO’s technical specifications, published on 31 December 2024, set out minimum quality, safety and performance requirements for oxygen sources, storage and distribution products used within health facilities. They provide a reference for qualified technical assessment; local approvals and the customer’s requirements must also be established.

JOTS analysis: define the installation and the payer

Choose one facility or a small cluster within a serviceable area. With the owner’s permission, obtain an asset register, manufacturer details, warranty conditions, maintenance history and downtime records. Have qualified biomedical and engineering professionals establish the service scope. Clinical decisions and technical work should remain with appropriately authorised personnel.

Identify who owns the equipment, who orders service and who controls payment. An installation funded through a project may have a different operating budget and decision-maker once handed over. Confirm the procurement route, invoicing process and payment timetable before committing staff or stocking expensive parts.

Build a deliverable maintenance offer

Define preventive visits, fault reporting, escalation, parts availability and documented service reports. Confirm manufacturer support, technician competencies and access to appropriate testing resources. Set response commitments against real travel times and staffing. Agree how the facility maintains continuity during planned work or breakdowns, with responsibilities approved by its technical and clinical leadership.

Price the full service rather than labour alone. Include travel, training, approved parts, test-equipment costs, insurance, documentation and working capital. Separate scheduled maintenance from emergency call-outs and major replacements. Identify exclusions and who pays for problems involving utilities or infrastructure beyond the agreed scope. Avoid promising continuous availability without a credible operating plan.

Actionable takeaway

Prepare a pilot proposal for one installation: verified asset list, baseline condition, named payer, technical scope, maintenance calendar, parts plan and acceptance measures. Track completed visits, fault-resolution times, repeat failures and service costs. Expand only after the customer and qualified technical team have reviewed the evidence and the payment arrangement works in practice.
